Dallas Permit Registration

I am applying for an alarm monitoring permit in Dallas County, TX. For Alarm Company Information, I need to enter company information for the following: 1. Monitored By 2. Sold By 3. Serviced By 4. Installed By I purchased my own system from Amazon and did a self-install, so I will choose "SELF INSTALLED" for 2 and 4. But what is the difference between "monitored by" and "serviced by"? Also, I have two Ring-related options to choose from in the available entries: 1. Ring Protect (AC105) 2. Ring Security (AC200) Which one should I choose?

https://support.ring.com/hc/en-us/articles/360029414032 I am in Fort Worth, and per the instructions in the above link, I believe we have to choose Ring Protect for all the fields.
